This form is optional and will be used to help students and families connect with each other for school projects, homework help, school events and more. Please complete a separate form for each child.

The information provided will be formatted and emailed to the email address(es) you provide and to others who complete this form for their student(s). You may list up to two parents/guardians. Please contact Linda, the Pfaff PTO Board Treasurer, at treasurer.pfaffpto@gmail.com with any questions or for help with this form.

Please submit forms by Friday, September 20 so that the completed directory can be sent out to participating families in early October.
